Instead, treatment for this child should start with modifying behavior and teaching both the patient and parent how to deal with concerning behavior. Reassuring the mother that this is normal behavior would be inappropriate, as this isn’t consistent with normal development (Answer A).

What about autism
He's just a boy. Boys need to spend hours in the outdoors using their muscles. Near the end of the day, when he is winding down, take 10 minutes a day for 12-16 weeks to teach him to read, write, and spell with alphaphonics. When kids can read, they become more focused–even an active 4-year-old boy. When they are focused, they settle down to read those beautiful picture books (especially about dinosaurs and trucks) for themselves. They no longer have to beg you to read to them. I have taught lots of normal (but seemingly ADHD) active 4-year-old boys to read, write, and spell. Some take longer, but be patient and do it with a sense of humor. Sometimes boys learn their lessons rolling on the floor, but they do learn. After they gain the skill, it changes them for life and sets them free. When the boy gets to school, he won't have a behavior problem. He will be the teacher's favorite because he's smart, focused, and well-behaved–although he will still roll on the floor from time to time. He's a boy!!!

He is 4, a boy and should not be expected to sit quietly and color like girls his age! Find a better school that has realistic expectations of how normal boys behave!

A&B. Unfortunately I was a young single mother with a five-year-old showing all these signs and more. These school and the doctors pushed for meds, and years later I would regret it. We never got behavioral therapy either.
The youngest I've seen adhd meds prescribed to is 3 yrs old. 😮

then maybe C. or, have the mother try a no sugar drink with caffeine, and just monitor the child's response. Children with ADHD are often calmed by caffeine, but it's very important to avoid sugar when doing this.

Needs a psych eval to get a proper diagnosis. ADHD, Autism, and trauma response behavior can all look very similar to someone not specifically trained in behavior and psych disorders. Referral to behavioral therapy won’t hurt either, but a proper diagnosis helps the therapist as well.

better diagnostics. With that description it could also be ASD. Or reaction on some sort of abuse. Or being highly gifted and highly bored. Btw which 4yo is at school?! And has exams?! I'd suspect a highly gifted Asperger's being deadly bored by his stupid surrounding (I have three of them at home). With your solution, to decide immediately on ADHD, you are risking his life. In USA, Aspergers have a suicidal rate of 50%, so you should be really sure he is not one, before you give him the wrong therapy or medication.
